Here are some notes to help you look at the options.
NOTE: UHC has just updated their plans. They still offer the “A” version below along with some new options. Once we evaluate the plans we will update the selection below.
- There are 4 basic plans. The difference between the standard and the “A” version is the max Benefit. The standard plans have a max benefit of $600K. The “A” plans have a max benefit of $2M.
- Deductible range from $2,500, $5k, $10k, $12,500. Plus Elite also has a $1,000 deductible option
- Max Coinsurance does NOT include deductible or copays. So your max exposure is the deductible PLUS max coinsurance PLUS copays.
- If there is an RX program, RXs are paid for at the Point of Sale, then submitted to the insurance company, which will go towards your deductible. If you hit your deductible, you will be responsible for the coinsurance.
- Coinsurance rages from 20%-40%, except for Plus Elite, which has no coinsurance.
- Value Select – Has no copays or Rx plan, can have coinsurance of $5K or $10K
- Plus Select – Has no copays, but has an Rx Plan. Can have coinsurance of $2K, $5K, or $10K
- Copay Select – has copays and an Rx Plan. There is only one option for coinsurance of $5K
- Plus Elite – has no copay, has an Rx Plan, but does not have any coinsurance, so you only have to meet your deductible. Plus Elite also has a $1,000 deductible option.
